Not All Contact Lenses Are Equal: A Low Vision Patient’s Guide to Better Choices

Responsive image

Low vision is a visual impairment that is uncorrectable by way of spectacles, contact lenses, medications or surgical procedures. Those with low vision find that certain activities of daily living like reading, recognizing faces, and/or driving may be challenging - even when wearing corrective lenses! Regardless of broken lenses or having previously successful lenses, low vision is a very real existence.

The information within this page describes the decisions one must make when selecting contact lenses for low vision. Choices are never made in haste as it pertains to low vision lenses. As a reminder, each eye is unique; therefore, a choice that is good for one person may not be good for another. When a person's vision is compromised, the ability to adapt to corrective lenses can be limited.

Understanding Low Vision and Why It Matters

Low vision refers to a visual impairment that cannot be fully corrected with glasses, contact lenses, medication, or surgery. People with low vision may struggle to perform everyday tasks like reading, recognizing faces, or driving even with corrective lenses.

Common causes of low vision include:

  • Macular degeneration
  • Diabetic retinopathy
  • Glaucoma
  • Retinitis pigmentosa
  • Optic nerve damage
  • Severe refractive errors

Because of the diversity in causes and severity, a one-size-fits-all approach doesn’t work. This is where customized contact lenses come in.

Why Standard Contact Lenses May Not Work

Traditional contact lenses are designed to correct common vision issues like nearsightedness or farsightedness. However, they may fall short for low vision patients due to:

  • Irregular corneal shape (as in keratoconus)
  • Scarring or opacities on the cornea
  • Extremely high or asymmetrical prescriptions
  • Need for visual magnification of image enhancement
  • Dry eyes and poor tear film

These factors mean that using standard lenses can result in discomfort, limited clarity, or no noticeable improvement at all.

Types of Contact Lenses for Low Vision

Thanks to advancements in ophthalmology and optometry, there are several specialized contact lens options for low vision patients. Each type offers unique benefits depending on your condition:

1. Scleral Lenses

  • Larger than regular lenses
  • Vault over the cornea and rest on the sclera (white part of the eye)
  • Ideal for keratoconus, corneal scars, and severe dry eyes
  • Provide a smooth optical surface and improved hydration

2. Hybrid Lenses

  • Hard center with a soft outer skirt
  • Combine the clarity of rigid lenses with the comfort of soft lenses
  • Suitable for irregular corneas or high astigmatism

3. Gas Permeable (GP) Lenses

  • Rigid lenses that allow oxygen to pass through
  • Highly customizable and durable
  • Offer sharper vision for certain low vision cases

4. Piggyback Lenses

  • A combination of soft and hard lenses (hard lens on top of a soft lens)
  • Helps improve comfort while maintaining clarity
  • Often used for patients with high corneal sensitivity

5. Prosthetic Lenses

  • Tinted or painted lenses designed for cosmetic or therapeutic reasons
  • Used when vision cannot be restored but appearance or light sensitivity needs management

What to Consider When Choosing Lenses

Finding the right contact lenses for low vision isn’t just about clarity it’s also about comfort, daily lifestyle, eye health, and long-term prognosis.

Before selecting a lens type, you and your eye care provider should evaluate:

  • Visual needs (reading, driving, digital screen use)
  • Corneal topography (mapping the shape and surface of your cornea)
  • Tear production and dry eye assessment
  • Lens material compatibility
  • Maintenance and care routine
  • Financial investment and insurance coverage

A proper lens fitting and trial period is essential before finalizing the choice. Follow-ups should also be frequent to ensure vision performance and monitor for any complications.

Red Flags to Avoid in Low Vision Lens Selection

  • Buying over-the-counter or online lenses without prescription
  • Ignoring lens discomfort or dryness
  • Using regular soft lenses for advanced corneal conditions
  • Skipping follow-up appointments
  • Wearing lenses beyond their advised duration

Empowered Vision, Personalized Choice

A good pair of contact lenses for low vision can be a life changer. Not only will your vision be improved, but your emotional health may improve too. With the right lenses, some independence may be restored you'll feel more confident and be able to return to your daily activities with ease.

To recap, here’s what low vision patients should look for:

  •  Custom-fitted contact lenses (not ready-made)
  •  Specialized lens types (scleral, hybrid, or GP lenses)
  •  Regular follow-up with a low vision specialist
  •  Prioritizing eye health and comfort over quick fixes
  •  Education about lens hygiene and care
